dear doctor,

i have a big issue with my health and i will give information about my health as much as i can.

i am a 28 years old man.

i was using aldara in for my warts, i did not want to bleed warst thats why i used hair removal cream on my genital area. after three days,i feel burning,tingling in all my genital area on october 10 and my urine was dribing when i sit or crouch . Then i went a doctor and he recommended me to get test. I had urine test, clemedia and hsv tests.

in my urine test mucous section-was many and colour of my urine comes amber. the other things were normal. but im not sure did they test me about any bacterial issue.

My hsv 1 comes positive and my hsv 2 igg comes negative. Actually itwas normal because i had oral herpes when i was a child.Then my doctor said your hsv 1 number is high thats why probably you were having gental herpes outbreak. By the way i did not have sore on my penis. I said okay and returned home. Pain last more than twenty days. Then 2 weeks(14 november) later pain came without any fever and it lasts one week than it went away. I had not masturbate for about three months than for my health i masturbated and i felt the pain in my penis. Then i did not mastubated till last week. After my last masturbation i started fever in my penis. When im sitting i feel the leaky feeling in my penis and my urine is foamy. Even when im taking shower when the water touch my penis i feel little pain there.

My last sexual intercouse was in may...

by the way i took the fda approved home access tests for hiv 1 after 18 weeks of my last sexual intercouse and it came negative i began to think that is my test is false negative. and also i havent been with anybody from west africa and i dont use drugsmy questions are :

1)The symtomps that im experiencing is really due to herpes, if not what they can be?

2)Can a fda approved test come false negative after 18 weeks, or should i take hiv 2 tes too?

3) what should i have to do?

ANSWER:

Welcome to the STD forum. I also looked at your discussion on the herpes community forum, where you had accurate replies.

Your doctor is wrong and apparently does not understand herpes infections very well. If your oral herpes diagnosis (as a child) was correct, you are immune to a new HSV-1 infection of your genitals or anywhere else. Your positive blood test for HSV-1 probably is due to your childhood infection. To your questions:

1) Your genital symptoms do not sound like herpes or any other STD. They are unrelated to sex. The changes you have noticed in your urine -- increased color, foaminess, strong odor -- probably are due to variations in diet and fluid intake; for sure they are not due to herpes or any STD. Penile pain without sores, blisters or discharge is not an STD symptom.

2) Your HIV blood test at 18 weeks is 100% reliable. It cannot be false negative. You do not have HIV.

3) If your symptoms continue or you are otherwise concerned, continue to work with your doctor. Or if not confident in his knowledge in this area, find another doctor. In the meantime, you should be having no worries about herpes, HIV, or any other STD.

Hi fever, well from what I have read I would have to mostly agree with the STD forum that replied to you. Is there a reason why you think you may have contracted genital herpes? Were you recently with a partner who you believe may have had herpes?

I don't know if I would go as far to say though that you are immune to GHSV 1 just because you already have OHSV 1 and I wouldn't be experimenting to find out either!

It is possible you are experiencing herpes symptoms. Not all outbreaks will have visible sores. Many member here only get tingling, burning and uncomfortableness for a period of time. I think it is safe to say you do not have HIV. Your doctor needs to rule some things out first with blood and urine tests before he can get to the bottom of what is ailing you. Your foamy urine could be anything from a UTI to a case of too much protein in your urine, which could be caused by kidney problems. You need to find a thorough doctor, one who is understanding of your concerns.

hi aquaruis angel, thanks for your message, im feeling the pain since three months ive seen 3 doctors. 1 of them was dermotologist another is a urologist and the other is a docto at school hospital.

i had a blood test and i wanted to get an swab test but they said when the next ob start they could do that. the problem is next ob did not come because my pain did not go completely away.

i thought i contracted herpes bc i felt burning sensation and tingling at first..(actually i used hair remover cream and aldara to that area and also apple cidar may be chemically smt happened to me) also one of the doc said my herpes 1 result is too high, and it could be herpes. still i have no sores,and the pain is not only one place when im touching my penis i dont feel a pain but even in the bathroom while taking shower water dropes giving me pain. i did what i can but nothing happened.

i started not to believe in doctors my life is really ruined.

It certainly sounds like you have all bases covered there regarding the medical treatment you have received. I am not sure how they are going to take a swab test though if there is no lesion to swab. You mentioned that even at your worst time of it, there are no visible lesions. As you have used hair removal cream in that area, it may have triggered some kind of skin reaction. Apple cider probably isn't such a good thing to use either on such a sensitive area of your body. You could just be dealing with a prolonged reaction to the hair removal cream and apple cider. Let it settle down for a while and see what happens. If your urine and blood tests have come back normal, then there is nothing to worry about for now. I would also suggest you increase your water intake daily, amber coloured urine suggests dehydration.

thank you very much for your valuable information, i hope its a skin reaction and it goes away but the pain make me anxious, i read some article about herpes zoster they say it gives sensation in the nerves and pain lasts longs even years, i hope i have nothing related with virus, i went every doctor as i said may be i should try a neurologist...

i will keep your advis and drink more water thank you angel again.

      Thanks @WilsoInAus I understand apologies for this but the only other thing I am worried about is that I did take a course of Famvir on day 1 as I felt like I was having an outbreak of cold sores and am worried that has suppressed the initial outbreak genitally

      Hi @sgt98 it is not feasible to obtain a HSV-2 infection from receiving oral sex - only HSV-1 is feasible but you've already got that and immunity from any further infection with HSV-1. You do not need any further tests for HSV. You're feeling regret, try to forgive yourself and calm down, let the rational take over. You know the answer here and it won't be long until you believe it too.

      Hi @NerdP423 and welcome to the website. You raise a lot of points and I think the best way to address your concerns is add some comments at each key point. I've copied your note again below and added these comments in square brackets. I appreciate any insight (even speculation) as to what has been going on with me. I'm a 37y/o M. I last was intimate with a new partner on 2/11/2022, and a few days later started having a huge amount of discomfort in my face. [The first thing that happens though is that herpes causes lesions or at least some skin based disruption. Other symptoms are then related to the actions of the immune system responding to the virus. Without lesions, it is highly questionable that the ailment is related to herpes, yet testing is useful if you have concerns as you have done so.] About a month after that [herpes causes issues within days, if the first 'symptoms' are a month later - its extremely unlikely they are related to herpes], significant discomfort downstairs, however every test I have ever taken for HSV 1 and 2 has been negative. Here are the details: Face: Previously, some significant tingling and itching on the right side of my mouth, lips and chin (still there, but milder) [herpes does not cause general tingling and itching, it can cause a reasonably concentrated feeling of itch/throb from which a herpes lesion appears within hours]. Occasional hot flashes near my right eye, cheek and ear [herpes does not do this, it may be a immune response to something, or stress]. Sometimes it will feel like the skin is crawling on the right side of my face [herpes does not cause a general crawling sensation]. Never seen anything that looked like a traditional cold sore [that's extremely telling, even people with associated atypical symptoms will have experienced herpes lesions]. Occasionally, the left side of my face will have a momentary feeling of skin crawling, but it's so mild that I am not really worried about it. Downstairs: Thankfully, most of these are now milder than they were before. Occasional momentary pinch of pain at the base of my genitals. [herpes does not cause a general pinch feeling.] Aching pain in my boxer area (groin, leg folds) [nor this] Occasional feeling of cold in my boxer area, butt, or lower back. ( also in my shins and occasionally even my arms) [nor this] On 4/30/2022, I had been in discomfort for almost two months. I scratched an itch, noticed it hurt, and then checked - I did have an open ulcer down there. Took myself to the ER to get swabbed, came back negative. [If this was a PCR then this will be very conclusive.] Practitioner said it may come back negative because it was already open. However the lab report noted that it definitely didn't look like what you'd expect a typical first herpes outbreak to look like [how would the lab know??]. All blood tests negative so far. I had two western blots, the second one was nine months after exposure. [Two negative Westernblots!!! Many that's real convincing]. All other swabs also negative. I went to urgent care, because I know the timeliness of when the Swab is taken matters. One time, a swab was not done, because practitioner said it was folliculitis and wouldn't swab it (It was at my belt line). [Belt line is highly unlikely to relate to herpes.]   A blood test revealed I had low-ish B12 (technically in range, but at the very low end, especially for a man of my size). I had a series of B12 injections, and I am taking a B12 supplement. I am not taking any lysine or arginine at the moment. I was taking the Arganine to see if I could induce an outbreak. [This is irrelevant, there is no known linkage between herpes outbreaks and arginine/lysine intake - its a myth - and B12 infers nothing.] I think what I am asking is - has anyone here ever repeatedly tested negative over and over again over long period of time, before getting a definitive answer, be a positive test, or something else? [The answer to this is: Extremely few people with a HSV-1 infection and even more rarely HSV-2 test repeatedly negative on Westernblot and actually carry the virus. Of the cases that I know of that had delayed detection by a swab and negative blood tests in the meantime (and that's only 3-4 cases), they had some form of lesions within days of infection but did not obtain a swab for various reasons and then obtained a positive swab of a subsequent lesion with the record being 11 months later. A couple of the cases did have some 'background' symptoms they thought might be related to herpes but that isn't ascertained and some did not have any unusual symptoms at all apart from the lesions. Hence as you did not have lesions around your mouth or lips within days of the last sexual encounter as is exceptionally common for a primary oral HSV-1 infection, that pretty much rules out herpes orally as it is. The fact that you had no genital symptoms for a month also rules out genital herpes. I am not aware of anyone at all who has genuinely gone on to test positive by swab or blood in your specific circumstances. There are hundreds if not more than a thousand experiences on this website alone that are similar to yours that are truly negative for herpes I'm one of them!]
