What could this be? Please help. Somehow diagnosed +HSV1 and HSV2

Good Evening everyone,

 

Looking for a bit of help after recently having an outbreak.  I am also still having some issues.  No clue what this could be.

 

Anyway, some details before the story...

After getting these symptoms, I somehow tested + (positive) for HSV1 and HSV2 only 1 month after  -  I have never had an outbreak my entire life.  (I am 30 years old)  (I also had never been tested before)

 

For starters, I recently had 2 sexual encounters with 2 different girls on holiday.  One was unprotected sex, and one was unprotected oral.

 

It wasn't until 3 or 4 weeks later that I began to develop symptoms.  (I will show pictures gradually progressing).  It started with tiny pimple like postules with a bit of redness around them on the shaft of the penis.  I immediately went to the doctor, because my immune system is weak due to taking a biologic medication and I was worried.  I was told it was likely herpes and given a 10 day course of Valtrex, 1g 2 times a day.  I was then told by my speciality doc to continue taking the Valtrex for suppression therapy sense my immune system may be weaker and I may have challenges fighting off the outbreak.

After taking the Valtrex for 2 days and putting hydrocortisone cream on the intial pimple like sores on the shaft, they somehow went away.  In the next two days, for some reason what looked to be a rash started to develop in my pubic hair, with some red spots spread out.   I started to have some tingling and itching, and what seemed to be something felt like it was crawling around down there.  I forgot to mention I was also having back pain before the initial pimples showed up.  Anyway, as things progressed the burning got worse, and 2 burn like bigger red spots persisted as I continued to take the Valtrex.

Within about 2 or 3 weeks, these symptoms began to die down, but I have had some itching and burning that persisted.  The original larger burning ones eventually have gone away, but now i seem to have what looks like a bit of a rash in my pubic area once I shaved to have a look.  I was also still getting red spots around the bottom of the penis every once in a while.  Over the course of this whole time, I had a tiny bit of pain when urinating.   Another note, I also had red bumps appear on my ass and on my tailbone that persisted a while and would not go away.  I also have red bumps still on my inner thighs.

I have seen multiple doctors as things have reoccurred to get further opinions as to what is going on.  I am now developing some pale white skin and some red spots or scab looking things on the head of the penis, as well as having a few clusters of tiny red bumps on the base of the penis.  My face has also started to break out with something I am unsure of.  They don't seem to be pimples, and they are like red and normal raised areas that do not hurt like pimples.  Two of them seem to be like scaly red patches that are uncomfortable with a white dot in the middle where the hair follicle is (above the lip).  Also, hair follicules seem to hurt.  The Dermatologist then gave me some bacterial antibiotic cream and told me to cleanse my face.  This didn't work.  My face was still itching.  I instead tried to use some ketoconazole shampoo and clotrimazole cream (anti-fungals) on my face because I thought this may be fungal,  The itching stopped, but most of the red spots have persisted.  I have never had acne before this.

Over this time, it has been approximately 5 weeks now.

 

I spoke with the girl I had sex with, and she somehow got what doctors thought was a severe yeast infection (She was given 3 antifungal type medications).  Her symptoms cleared up.  She has not been tested for Herpes (I have told her to get tested even though it is unlikely to show up, I think?)

 

At this point, I am completely unsure of what is going on and what this could be.  Could this be an initial herpes breakout?.. not sure if hsv 1 or hsv2?  Why did I never have one until now?  It also could be some type of yeast infection that I never got  treated over the past 6 weeks?  Maybe its scabies?  Maybe balantitis is starting to develop?   I'm really not sure.  

 

Any insight you all could provide would be greatly appreciated!

Hey @StealyMan hope you're going well.

The first and most important comment to make is that these particular symptoms look nothing like herpes lesions (on your penis) - this is independent of your herpes status, see below.

I feel a bacterial cause does top the list, it may even be staph for example that could be quite hard to get rid of. I can't see anything further now genitally for you so that has probably cleared away.

A key contributor here can be poor diet, did you slide into bad habits on the holiday? It seems like having more vegetables and water but less oily foods might be of benefit.

All these symptoms are independent I suggest of your HSV status. In order to assess that a little further, you will need to understand the exact test you had and what were the results. You're looking for whether it was IgM or IgG and the values relative to the positive cut off point.

Thank you very much for the quick reply.  I really appreciate it as I am heading back to the Dermatologist tomorrow.

 

If the pimples did not look like any type of herpes, did the burning lesion type things in the pubic area look like it?

 

It's very possible that it could be a staph infection I guess  (does staph act like this?).  I cut my foot open and used some antibiotic ointment for it a while back.

 

I control my diet very very well when I am at home and working.  When I am on Holiday, that is not the case.

 

In regard to the tests, I had blood tests done which showed high antibodies to both HSV1 and HSV2.  I am not too sure much else on that.

 

Do you think these symptoms could possibly be a yeast infection or some candida invasive problem.  Or, is it possible for it to be anything else?

@StealyMan you do need to obtain the test results. Some tests like the IgM are not diagnostic. IgG tests can be combined for HSV 1/2 which means either or but not necessarily both infections.

What you are looking for is a test result for IgG HSV-1 separate from IgG HSV-2. 

Bacteria or yeast seem a pretty high chance here, but good to be seeing a dermatologist, good ones are magic.

The pic from your groin area also does not look like herpes.

      Yes, but every married person who I found out about that has this waited 6-8 months into the relationship to disclose it. But maybe you're right. If I had told her 6-7 months in, she'd still have Googled it and flipped out, and maybe it would have been harder then. I don't know. I don't see myself going through this level of pain and rejection so easily next time. I really don't. I'm taking the meds. I use protection. It's been almost a decade since I've had it so I'm not worried about shedding or passing it on so easily. British studies confirm that the first 2 years are the most contagious and we're passed that. I'm just over this. I've never been in so much emotional pain in my life.

      Is this herpes? I can’t find any pics that match this exactly and my symptoms don’t really match. I don’t have really any of the typical herpes symptoms other than pictured, so I figured it could be friction as I have recently been having a lot of sex with a new partner. Way more than normal. Started having unprotected sex about 3 - 4 weeks ago. These symptoms only started about 3-4 days ago. Felt burning in some areas and only some very few tiny red areas/spots appeared. Almost shiny in appearance. Seems to settle very quickly. They were only present for a day or so. Also had some very mild irritation after I pee but not during. This spot has 2 small red lumps that are now dry around the outside. It’s like it’s been rubbed off and the skin is dry and shiny underneath. It doesn’t seem to be lesion. There was no puss or any leakage, nor was the skin ever red under them. They also got better within a day or so. I still have red spots around but they are very small and not bunched together and some general discomfort but I feel it’s because I’ve touched it so much! Stressing a bit!  my research tells me friction burn/blister , Fungal ,herpes a bit of it all! I have no idea! Please help! Thanks in advance.

      @EssenceL25 there really isn't a question mark around the source, it's your latest partner. The median time from infection to an outbreak of lesions upon initial infection is 4 days.  The only thing that isn't certain is the type I suggest.
